This site was created by two Registered Nurses, who have over 60 years of combined experience in Nursing, including MDS Coordination, Nursing Home Nursing, Rehabilitation, Med/Surg Nursing, Psychiatric Nursing, Chemical Dependency Nursing, Home Health Nursing, and Medicaid-Waiver Program for Aged And Disabled Nursing.

In our many years of nursing we were frustrated with the “cookie-cutter” style of nursing care plans available and the lack of resources for individualized care plan guides. The goals of this site are:

 

· Provide interactive instructive samples of nursing care plans addressing problems  unique to nursing home residents

 

· Decrease the man hours of developing individualized plans

 

· Decrease “burnout” of the MDS Coordinator

 

· Decrease the employer’s turnover rate of MDS Coordinator positions.
