Altered level of cognitive function due to dx of

Resident has memory problems, impaired decision making skills and impaired ability to comprehend Also has difficulty understanding what is being said as well as being understood

Resident will remain oriented to

x 3 months.

Resident will be able to make basic needs known on a daily basis x 3 months.

Call resident by name with each interaction in order to reinforce name recognition.

Introduce yourself and explain what you are going to do in plain simple terms.

Keep routine consistent and try to provide consistent care givers as much as possible in order to decrease confusion.

Present just one thought, idea, question or command at a time.

Break tasks into one step at a time.

Ask yes/no questions in order to determine resident's needs.

Use plain, simple, concrete language when speaking with resident.

Cue, reorient and supervise as needed.

Supervise and assist with all decision making.
